The first visit is a time to really start to get to know you. This is done through paperwork, and then more critically one on one with a Doctor of Physical Therapy. The evaluation is the time to really be heard as far as the history of your injury, and to fully understand what is going on and what may have caused the injury or discomfort. Therapists then perform targeted testing to further their fact-finding into your problems, and confirm any diagnoses referred by your physician, and/or add therapy diagnoses from our findings. This detailed look guides us to develop an individualized plan of treatment. This is also a time for the patient to ask any questions they have and get a better understanding of the findings from the evaluation, while also inputting their thoughts and any treatments they may have self-tried or received from any other persons.

Comfortable gym clothes are highly recommended; think clothes you would work out in that will not restrict any motions. We highly recommend loose-fitting clothes so that the body part we are working on can be exposed to the skin level for any necessary assessments and treatments. Comfortable shoes such as tennis shoes are recommended to tolerate activities, and provide the support and grip needed for exercise activities.

We schedule each appointment for an hour slot. A typical appointment could take anywhere from 45 minutes to an hour and 15 minutes, depending on specific treatment plans and modalities/treatments required.

This is highly insurance specific. For any Medicare patients, a physician’s referral or script is required as per Medicare rules. Medicare Advantage plans differ in this area and most do not require a referral, so it is best to consult your plan, or contact our friendly front desk staff who can assist you in obtaining this information. Most commercial plans do not require physicians’ referrals, and again this is insurance specific. Our cash-paying patients do not require a referral to initiate therapy services with us.

We are in network with a number of insurances; a list of which can be located on our website or status can be obtained by giving us a call. We also frequently see patients out of network for those that have out of network benefits. Patients can then obtain a bill to submit to their insurance for help with coverage. In the case of some specific insurances, we will bill your insurance out of network directly on your behalf. To see if your insurance is one of these, you can contact us directly. As we strive to build our insurance base and try to expand the networks we contract with, we offer special pricing on cash rates to help with this process. In most cases, treatment out of network allows you to get superior care as compared to going in network and being lumped in and seen in session with multiple other patients, or solely by assistants, which heavily affects recovery rates and times. Coming to Sciform Physical Therapy allows you to get Doctor of Physical Therapy level care and more one on one time, leading to better results and quicker recovery.

We do accept workers compensation patients. Take a look at our list of networked workers compensation companies. Most patients who are receiving care under workers compensation do not know that you can choose where you do your workers compensation physical therapy as long as they are in network. Patients are generally shuffled through corporate owned physical therapy clinics with high volume and limited slots, which means patients receive inadequate care due to therapists treating multiple — in some cases more than three patients in an hour. Patients spend large portions of their therapy time waiting for the therapist to free up to give further instruction, or their form with activities is not fully observed. These issues cause slower recovery rates, increased out of work time, and lost wages or work hours on behalf of employers. Yes, we offer cash options for patients. This option cuts out the burdensome requirements of the insurance companies, allowing the therapist to be fully engaged with you throughout the entire session and not taking time out to make sure all the documentation needed to get paid by an insurance company is worked on. Cash patients also get great flexibility in the session, which in the case of certain insurance companies would not be possible due to strict rules they have, or even prevent you from getting treatment techniques that they do not approve of. Take full control of your rehabilitation program and cut out the middle man.

We frequently see patients who have prescriptions with another therapy clinic’s name on them. These scripts are created as a way to push patients to specific places — in a lot of cases, to clinics in which the referring doctors or the companies that own the offices also own the PT clinics that they are sending you to. In these cases, while there is convenience of medical records and flow of documentation, patients have the notion that doing therapy at their doctor’s clinic or the place they recommended means that their doctor is keeping a keen eye on progress. This is NOT true. Often patients also get inadequate therapy as these centers do not have to work hard for patients because a steady stream of patients with good insurance flows into their clinics. Copays and coinsurances are dictated by your specific insurance plan and not by Sciform Physical Therapy. Talk to your insurance to confirm these amounts prior to starting therapy and see if it fits your budget. Knowing these numbers ahead of time will help in the creation of your plan of care at Sciform Physical Therapy, where our Doctors of Physical Therapy can work with you to set up a program that will maximize results while also being very sensitive to your budget.

Generally, patients are seen twice per week in clinic. We focus on developing and updating a home program for you to work on when not in clinic. Post-surgical patients depending on the procedure may require three visits a week, and some patients with minimal pain, less complex cases, or that have financial problems can be seen once per week with a heavier focus on home programs. Here at Sciform, the number of visits is not dictated to the patients but takes into consideration all the factors and is decided alongside the patient.

This is dependent on the problem/injury or surgical procedure being treated. Typically patients visit us for 8–12 visits, with the average patient completing 10 sessions of PT to treat their issues and leaving with an appropriate individualized program to follow at home — both a written handout with pictures and access to a handy app that has all their program in video form to help with accuracy. Compared to other therapy places, here at Sciform PT you are seen by a Doctor of Physical Therapy, allowing for faster recovery with fewer visits required in total as compared to our competitors, where you may only be seen once or twice by the evaluating therapist and then bounce around different therapists for the remainder of your sessions, which leads to slower recovery rates and more expenditure on copays and coinsurances over the course of your total therapy program. Therapy programs at Sciform Physical Therapy are also not rushed to free up schedules, which is seen at high volume clinics that have more referrals than they can handle, leading to long wait times and patients being discharged from therapy earlier, which inevitably affects end results and increases the likelihood of re-injury or recurrence of pain.

We do not have online payments available at this time. Payments for copay/coinsurance are taken at the clinic at the time of session; this helps prevent fraud or the need to store patient payment information, protecting both the patient and Sciform Physical Therapy. We accept all major credit cards, checks, and cash, whichever is convenient to the patient.

Give us a call or email us as soon as any changes need to be made. Patients that do not cancel within 24 hours of their visit will still be responsible for the cost of the session.

We prefer our patients come in to the clinic for their therapy sessions, allowing them and the therapist to have access to all of the state of the art equipment to best treat the patient’s conditions. Patients seen in their home are limited in the activities, modalities, and techniques a therapist can perform due to limited equipment. These patients do not meet their maximum potential and their overall functioning is affected long term due to this. Home therapy is for those that absolutely cannot get to a clinic. We at Sciform do have some of our cash-based treatments that can be performed in a patient’s home on a concierge basis. All home treatments and therapy are cash only, and insurance is not accepted for these services. Contact us to find out more to see which works best for you.

Every patient is assessed and a personalized program is developed based on individual goals and needs. We offer both written programs and access to online applications whereby your program can be viewed with each activity seen in color video for better form and accuracy. These are routinely updated as your program progresses.

Patients are provided with home exercises to perform as early as the first session. While some of these exercises are still performed in session, they are given and updated routinely to help a patient progress to their personal therapy goals. Therapists will guide the patients on which exercises to continue and any that are no longer relevant for the patient’s individual program needs. The patient is responsible to continue these as prescribed while communicating any difficulties with activities during sessions with the therapist. We work together with you to help you achieve goals set out, be it for pain or function — one does not work well without the other.

Most of us do not adhere to dedicated workout routines to fill the gap in missing activity created, and those that do have routines that target only certain large muscle groups. Injuries or repetitive strain from poor form affect even the most avid workout enthusiasts. Physical therapy can help all of these people — whether it’s fixing posture, getting help with adjusting your workstation from an ergonomic expert, or figuring out why certain gym activities are difficult, not improving, or causing pain. Everyone can benefit from therapy.

After many years working in corporate owned clinics — the largest therapy providers in the country — our founder realized that some places focused on volume rather than quality. This meant therapists were pushed to see multiple patients at the same time for almost every hour of their day. Patients were passed around due to the need to get patients on the schedule and try to keep up with the volume of referrals coming into each clinic. While some patients were happy with treatments and therapists, many were left less impressed, and worse yet, some thinking that therapy just did not work and was a waste of time. We at Sciform are guided by our principles of pride for our profession and the life-altering effects it can and should have on every single patient that comes into our clinic. We are rewarded by the successes of our patients in their goals for therapy and never want anyone to say they gained absolutely nothing from their therapy sessions.
